2. The completed Application form. (9 pages)
3. Personal Statement.
4. Statement of Financial Resource for Graduate Study.
5. Address Labels, Acknowledgement of Application and Notification of Comple
ted Application postcards. (Unfortunately neither US postage stamp nor inter
national coupon is available here. Please inform me how I can pay the postag
e.)
6. Three sealed and signed letters of recommendation from Prof. JIN Sizhao,
Prof. TIAN Lilin, and Associate Prof. WANG Yan.
7. Official Transcript from Tsinghua University.
8. Photocopies of GRE/TOEFL/TWE scores. (Official scores from ETS have been
ordered.)
9. A brief resumes containing my information of awards and research experien
ces, etc.
10. A photocopy of the page Final Results of the 28th IPhO.
11. Photocopies of my awards in Tsinghua University. (Chinese version only.
English certificate is not available in Tsinghua.)
12. Mailing Labels.
13. The form Report on Proficiency in English will be sent directly by my En
glish teacher YUE Zhenwu.
